Please be sure to make the signs and symptoms for your patient realistic but not completely obvious – part of the assignment is to consider other diagnostic possibilities based on your case.
This assignment should be written so that it is somewhere between 5 -10 pages in total
Write your paper in APA format. Websites like Wikipedia, Cleveland Clinic or WebMD are not credible references, but they may be good places for you to look for suggestions on important papers that have been published.

The Fast five quiz will be created from the content of your case study – it should be progressive and by that I mean you might start with a question regarding the signs and symptoms or the differential diagnosis and progress to the investigation (examinations, tests etc.) and then to pathophysiology, treatment and prognosis. Remember for each of the questions, explanations for why the answer you have chosen is best and why the answers are incorrect are to be included (as shown in the example).
The basic structure of the Case Study should include the following components:
• Patient’s Chief Complaints
• History of Present Illness
• Past Medical History
• Family History
• Social History
• Medications
• Physical Examination Findings (including vital signs)
• Laboratory Test Results (e.g., urinalysis, chest x-ray, or electrocardiogram if applicable)
Disease Summary is presented in the following order:
• Definition of the medical condition
• Prevalence of the medical condition in Canada with a focus on age, gender, and racial/ethnic group predispositions
• Significance of the medical condition with emphasis on bothersome symptoms, mortality rates, and national economic implications
• Causes and Risk Factors of the medical condition
• Pathophysiology of the medical condition based on established research findings
• Diagnosis: (based on) Clinical Manifestations and Laboratory Tests
• Appropriate Therapy with a focus on the primary goals of treatment and common nonpharmacologic, pharmacologic, and surgical approaches
• Serious Complications and Prognosis (i.e., the consequences of failure to treat the condition appropriately)
Be sure to be consistent throughout your report with respect to your patient you have described in your case study.
The major objective of this case study is to provide a current and close correlation between basic principles of human disease and clinical practice. It is important for students to thoroughly understand the underlying pathophysiologic processes that are present in the patients they will serve. Additionally, understanding basic pathophysiologic mechanisms of human disease ultimately promotes better decision-making efforts by healthcare providers and a better quality of life for their patients.
Goals for this assignment: Students will:
• Strengthen their vocabulary in human medicine, pharmacy, and nursing;
• Develop an ability to recognize clinical signs and symptoms that are consistent with a large group of common human health conditions;
• Develop an ability to identify abnormal clinical laboratory test results that are consistent with a variety of human health disorders;
• Develop clinical problem-solving and critical thinking skills;
• Gain insights for specific treatments and methods of clinical assessment.
Each case study should include a disease summary including definition, prevalence, significance, causes and risk factors, pathophysiology, diagnosis, appropriate treatment, and serious complications and prognosis.
